Vertical Lip Lines: Botox or Filler or PRP? Which Treatment is Best for Lips?
Vertical lip lines are notorious for giving away a person’s age. What’s more, everyday solutions to cover them up just don’t work. For instance, have you tried makeup or lipstick? The truth is they both often make lip lines look even more pronounced. Or, perhaps you’ve invested a small fortune in topical skincare? Unfortunately, all the creams and serums in the world won’t be strong enough to reverse lip aging significantly. So we’ve just given you the bad news. The good news? There are several top nonsurgical treatments that are great to get rid of lip wrinkles. In today’s post, Vertical Lip Lines: Botox or Filler or PRP, we’ll help you find the best procedure for vertical lip lines. Whether it’s Botox vs Filler vs PRP for lips, there’s a solution that’s right for you.
What are the Different Types of Vertical Lip Lines?
The treatment of choice will likely depend on the type of lip wrinkling and skin aging that concerns you. Types of vertical lip wrinkling include:
- Lipstick Lines are the parallel vertical lines that run up and down the length of the upper lip and/or lower lip. These types of lip wrinkles are often fairly shallow and fine, but do deepen with age.
- Smokers Lines are the parallel vertical lines that fan out from above the upper lip and/or below the lower lip towards the nose and chin, respectively. The medical term for these wrinkles is perioral lines. They are called smokers lines because they are caused by repeatedly pursing the lips, akin to smoking a cigarette. Although they start as shallow fine lines, they are typically deeper than Lipstick Lines. Like Lipstick Lines, they do grow deeper with age. They are often confused with Marionette Lines, aka “Puppet Lines”, which form parentheses on the jaw from the outer corners of the lips to the chin.
What is the Best Treatment for Vertical Lip Line Treatment?
Before we talk about the differences between Botox and Filler and PRP for lips, let’s talk about their similarities. Each procedure is:
- A top nonsurgical treatment to reduce lip wrinkles and fight aging
- Offers minimal downtime, risk and recovery, unlike plastic surgery or lip augmentation
- Provides results quickly after treatment
- A temporary solution and requires a treatment schedule to maintain results
How Do I Get Rid of Lip Lines? What is The Best Procedure for Lip Wrinkles?
We suggest three different treatments for vertical lip lines. Here’s more on each treatment, including indications, treatment and results.
Vertical Lip Lines: Botox
What Botox Lips Benefits Can I Expect?
Botox is used to get rid of Smokers Lines. As we said above, this type of wrinkle is caused by repeatedly pursing the lips. Botox injectable is excellent for any type of wrinkle that’s caused by expression. Smokers Lines are no exception.
What is Botox Treatment Like?
A small amount of Botox Cosmetic will be strategically injected around the upper lip and/or lower lip, in order to cause the muscle to relax.
What Botox Results Can I Expect Before and After? How Long Does Botox Last on Lips?
When the muscle is relaxed, lip lines will smooth out. Botox results are typical after 3-7 days. To maintain results, treatment must be repeated, typically in 3-6 months.

Vertical Lip LInes: Filler
What Lip Filler Benefits Can I Expect?
A dermal filler can be used for both Lipstick Lines and Smokers Lines. Specifically, we always recommend a Hyaluronic Acid (HA) filler for lip lines. That’s because HA is a naturally-occurring substance in the body that keeps the skin plump and hydrated. In particular, we recommend Restylane Kysse Lip Filler. The hallmark of Restylane Kysse is its smaller particle size, which makes it ideal for fine, detail work on the lips. In addition to reducing lines, a filler can meet other beauty goals for the lips, including:
-
- Increasing lip size — Hyaluronic Acid is a natural volumizer.
- Adding symmetry — Oftentimes, clients with thin lips complain that either the upper lip or the lower lip is thinner than the other. A Hyaluronic Acid filler can add balance so that the mouth appears in proportion.
- Changing the shape of the lips — A filler can change the contour of the lip. For example, a filler at the corners of the mouth can widen a small mouth. Or product in the cupid’s bow can add more pucker to the lip.
What is Lip Filler Treatment Like?
Like Botox, the doctor will inject dermal filler at points along the wrinkled area with a fine needle.
What Lip Filler Results Can I Expect? How Long Does a Lip Filler Last?
An HA Filler will hydrate and plump the area. As a result, volume-loss will be restored and lip lines will fill in. Filler results are immediate. To maintain results, treatment must be repeated, typically in 6-12 months, depending on the filler.

Vertical Lip Lines: PRP Microneedling and/or Injections
What PRP Lips Benefits Can I Expect?
PRP can also be used for both Smokers Lines and Lipstick Lines. The main benefit of PRP is that it works both on the surface and deeper layers of the skin. Specifically, Microneedling with PRP will repair surface imperfections. On the other hand, PRP injections will spark collagen production to reverse skin aging under the skin. In addition to smoothing wrinkles, PRP can work to:
-
- Improve skin texture on and around the mouth
- Redensify the skin on the lip area, in order to create a natural plump
What is PRP Treatment Like?
PRP involves a blood draw from you. Then, the platelet rich plasma is extracted from the blood. This highly reparative part of the blood is then injected and microneedled on the treatment area, in order to heal and rejuvenate the skin.
It’s important to note that we rarely recommend using PRP solely on one small area, such as the lips. In order to take full advantage of the healing factors extracted from your own blood, we would typically recommend doing the lips as part of a PRP Face Treatment or on another area of the body, such as PRP for chest or neck.
What PRP Results for Lips Can I Expect?
Some PRP results are immediate and include a reduction of fine lines and wrinkles, a healthy glow, as well as smoother and more youthful skin. Over the next few months, collagen growth will promote further skin renewal and regeneration. To maintain results, we typically recommend PRP for lips treatment once a year.
